Why do we have two types of breast implants: textured and smooth?

The main reason textured and smooth breast implants exist is to give people options based on their needs and preferences. When a breast implant is put into your body, your body’s natural response is to surround it with a layer of scar tissue. This is a normal thing your body does whenever there’s something foreign in it. This layer of scar tissue is called a capsule, and it can actually help to keep the implant where it should be.

Now, let’s talk about smooth implants first. With these, the scar tissue or capsule doesn’t stick well to the implant’s outer shell. This means the implant can move a bit, which some people like because it makes it feel and move more like natural breast tissue. The downside is that, occasionally, the implant might move too much over time and end up not where it’s supposed to be.

On the other hand, textured implants have a surface that helps scar tissue adhere to them more . This helps keep the implant in place, reducing the likelihood that it will move or end up in the wrong place. You may have heard of issues like implants "bottoming out" or moving sideways— implants help to prevent this.

Medical research has shown that textured implants placed above the muscle may also have a lower chance of a problem called ???capsular contracture.’ This is when the scar tissue around the implant tightens too much, making the breast feel hard or even look distorted, and sometimes causing pain.

Does the texture of breast implants really make a difference in the chances of getting capsular contracture?

Our team of surgeons is deeply committed to staying up to date with the latest medical research to provide the best care and results for our patients. They’ve been curious about whether older research on implant texture and risks still applies, especially given our modern understanding and surgical methods.

To answer this question, they’ve been conducting a new study using data specifically from Centre for Surgery. Previous studies sometimes had small numbers of participants or multiple surgeons, which made it hard to pinpoint what was really going on. Those studies also often dealt with an older generation of implants.

Our team took a fresh approach. They reviewed data from hundreds of patients who had breast augmentation surgery with us from 2016 to 2022. In this group, about a third had smooth implants, and two-thirds had textured implants. All of these were made from cohesive gel silicone. Out of all these cases, only five people with smooth implants and three people with implants experienced capsular contracture. After this data, they found that these rates weren’t statistically significant.

This means that whether the implant is smooth or textured doesn’t seem to significantly affect the likelihood of developing capsular contracture. What seemed to matter more was how the surgery was performed, particularly steps taken to limit bacterial contamination around the implant.

Other Risks Linked to Textured Breast Implants and Why We Often Choose Smooth or Microtextured Options

Since around 2018, our team of surgeons has been leaning more towards using smooth or microtextured implants. One primary reason for this change is our ongoing study and awareness of a rare form of cancer called . This uncommon type of immune system cancer can develop in the scar tissue that forms around a breast . The good news is that, in about 95% of cases, this cancer is treatable without the need for aggressive treatments like chemotherapy.

We’ve been monitoring the research, which shows that most cases of BIA-ALCL are linked to a specific kind of textured implant. This information has become more evident over the last few years, leading many UK to stop using this particular type of textured implant due to its connection with increased risks.

Interestingly, no known cases of BIA-ALCL have been reported in the medical literature involving only smooth implants. This was a significant factor in our decision to phase out the use of rough-textured implants altogether.

If you’re someone who’s had breast augmentation using rough-textured implants, it’s important to note that the risk of developing BIA-ALCL is extremely low. In fact, the minor risks associated with undergoing another surgical procedure to replace them are generally considered greater than the risk of developing this rare form of cancer. Therefore, most medical advice suggests that leaving the existing implants in place is likely the safest .
